For over 25 years, Healthy Family Partnership has implemented the nationally-recognized, evidence-based Nurse-Family Partnership curriculum to empower mothers to have healthy pregnancy, healthy babies and be great parents! Over four decades of research shows that Nurse-Family Partnership succeeds at its most important goals: keeping pregnant women healthy, keeping children healthy and safe, and improving the lives of moms and babies.
Healthy Family Partnership's specially-trained Registered Nurses regularly visit moms-to-be starting in the pregnancy and continuing through the child’s second birthday. During this visits, they address both immediate health needs and future life goals, such as education and employment. By providing a consistent, credible source of support, we help parents overcome barriers to success and promote healthier families. As a Healthcare Provider, you are a vital link between pregnant mothers and these beneficial services.

How do we support your patients?
The education, support and guidance the nurses provide surrounding pregnancy and childbirth are vital parts of our services. But we take a holistic approach to our services, looking at other areas were we can support mom and her families as they prepare for the new arrival and after baby arrives. completing regular screenings for mental health concerns and, when necessary, referring to our in-house Licensed Therapist who specializes in maternal mental health.

connecting to community resources that may be helpful such as emergency food and shelter assistance, domestic violence or substance abuse counseling, GED or job training programs.

completing regular developmental screenings to be sure baby is developing on track and referring to Early Invention services as needed.

ensuring baby is transported as safely as possible with a new infant carrier car seat installed prior to delivery.
What is the cost?
Healthy Family Partnership services are funded by State and local grants, making all services completely free to families. For some visits, we are able to bill Medicaid to offset some costs. Should a mother be enrolled in MA, we will contact your office to issue a prescription for services for billing purposes.
Referring to Health Family Partnership
We invite referrals for all pregnant moms in Wyoming and Sullivan Counties! It is not necessary to pre-screen for "need". While we do have moderate income guidelines for enrollment and, of course, those with high-risk pregnancies can especially benefit, we are all-inclusive! All mothers can use extra support during this time and we are committed to helping every mother who is interested in these free services. When we receive the referral, we will contact the patient and establish her eligibility. Depending on circumstances, we can waiver those income guidelines. If she, for some reason, she is not eligible for our services, we will connect her to resources that will be helpful.
